Staff help participants manage their physical and mental health needs so they can stay healthy and safe at home.
What does this support include?
Medication Management
Assisting with self-administered medications, keeping tracking logs, and making sure prescriptions are refilled.
Medical Appointment Coordination
Scheduling doctor, dentist, or therapy visits and making sure there's a way to get there.
Dietary Monitoring
Helping participants follow specialized diets prescribed by a physician or nutritionist.
Who can receive this adult disability service?
Adults with developmental and intellectual disabilities living in Idaho Falls and surrounding communities. We coordinate with your state case manager during intake.
